Pale and skinny?

Not very attractive they say but... what's attractive is pretty subjective most of the time.
Far from pale and skinny
really?

what are they supposed to look like?

As Vageenka said, for me it makes the person look serious, on a mission, ambitious, hardworker and responsible.

Not very attractive I would put an square aspect as the trine is an harmonious one.

they seem great but not attractive?

Square is a tension...So Saturn would be in tension with the ascendant energy. Saturn would put limits on one's ascendant, possibly making the person an introvert or shy. Maybe it put barries on one physical appearance.

Oh my God it's so crazy how you said it makes the person look serious, ambitious and responsible.

Emma Watson is such a good example of that. She has always come across super responsible and mature for her age in interviews. Mark Ruffalo has that mature presence thing about him too. Blake Lively as well, you never catch her drunk or having a slutty moment - she probably does it behind close doors or gathers herself together before going public in that state. Lol.

There are other aspects too...

I have Saturn trine my ASC but I do have Venus in the 12th house, so Im a little more relaxed behind the scenes.

Maybe, like a Cap rising, Saturn may provide good bone structure?

From skyscript:

The manifestation of Saturn in physical description is described as 'cold and dry'. This points to little flesh on the bones, small eyes, no alluring softness in curves and dimples, the hair is not soft, shiny and bouncy but sparse and tends towards being lacklustre, pale or dull. The colours of Saturn are those that want for the vibrancy of additional hues, being typically dark and black (Saturn's contact can add an element of darkness to other colours), white and pale, or a grey, ashy colour. The complexion and general appearance is often defined by this, being either pale and somewhat insipid, or dark and brooding. Saturn suggests a medium height but it is not a planet that by itself produces beauty or proportions that are pleasing to the eye. The body is generally thin and angular with prominent bone structure, but sometimes it may incline towards excess fat, due to a weak, lethargic constitution. The countenance is downward looking and there is often an awkward gait, a slow, hesitant movement with little trace of a spring in the step.

Of course the ASC would take priority followed by any planet placed in the 1st house and the position of the ruler.

Saturn is the ruler of the bones, so a Cap rising would have a good bone structure (if well aspected) or problems related to the bones (if bad aspected)

Cap suns or rising tends to have a strong jawline.
