1 of my friends said that her & her girlfriend (yes they are lesbians) are into S&M

She said they use knives during sex.

I've done this on occasion. It sounds crazy at first, I know it did to me too, but really I don't think it's as big of a deal as the imagination makes it out to be. There is a great amount of trust and care to be into account though.

I get the "rush/adrenaline" aspect of S&M or sex.

What I have trouble relating to is the "PAINFUL" aspect of it all.

Public sex gives me a rush, yes, & is great! lol but it's not painful! 2 different things!
