Given the health conditions that can occur if you allow periodontal disease to continue, the very slight risk associated with anesthesia is well worth it. Periodontal disease CAN and WILL cut off up to about 6 years off your dog's life. People don't realize, but the combination of bad nutrition, over vaccination, and bad dental hygiene is cutting our dog's lifespans in HALF. The good news is, you can change all of this for your dog, simply by doing the following- feed the dog raw food (www.barfworld.com) and only vaccinate with the puppy series, and every 7 years after that. The raw food will cure the periodontal disease because the simple act of eating the raw bones will clean the teeth. You're no longer feeding your dog poison. And same with the vaccination, every year you give a 'booster' you are NOT increasing the immune system's ability to fight off the virus, but you ARE injecting foreign substances into your dog. (Mercury for one thing)

Ok got on a slight rant there. Basically, descale the teeth to begin with, and from there give the dog a LOT of raw bones (one per day) to scrape the plaque off. Bones are how dogs brush their teeth. Raw bones in particular have a slight amount of meat which will keep the dog interested, and they have to chew it over and over, scraping their teeth on the bone, to get the meat off. Cooked bones are dangerous, and manufactured bones have too much starch and increase the growth of bacteria. Honestly, if you do the raw bones, you can skip the descaling.
